ارزیابی بیماران آسیب‌دیده مغزی بزرگسال فارسی‌زبان از منظر اختلالات کاربرد‌شناختی

چکیده
هدف پژوهش حاضر، ارزیابی مهارت‌ ارتباطی کاربردشناختی در بیماران بزرگسال آسیب‌دیده نیمکرۀ راست و چپ فارسی‌زبان با استفاده از نسخه فارسی «پروتکل ارزیابی مهارت‌های ارتباطی مونترال (ام. ای. سی.)» بود. ۱۰ بیمار آسیب‌دیده نیمکرۀ راست و چپ فارسی‌زبان که دچار سکته مغزی شده بودند، جامعه آماری پژوهش را تشکیل می‌دادند. دو دسته آزمون‌ در پژوهش حاضر مورد استفاده قرار گرفتند :آزمون‌های غربالگری و خرده‌آزمون‌های مهارت کاربرد‌شناختی پروتکل ارزیابی مهارت‌های ارتباطی مونترال (ام. ای. سی). عملکرد بیماران آسیب‌دیده نیمکره راست و آسیب‌دیده نیمکره چپ در این خرده‌آزمون‌ها به صورت انفرادی، درون‌گروهی و بین‌گروهی با یکدیگر مقایسه و با استفاده از روش‌های آماری توصیفی و استنباطی تحلیل شدند. یافته‌ها نشان دادند که بین عملکرد دو گروه بیماران آسیب‌دیده نیمکره راست و آسیب‌دیده نیمکره چپ در خرده‌آزمون‌های مهارت‌ ارتباطی کاربردشناختی تفاوت وجود دارد، اگرچه بر مبنای تحلیل‌های آمار استنباطی این تفاوت عملکرد معنی‌دار نبود. نتایج حاکی از آن بودند که آسیب به هر دو نیمکره راست و چپ مغز منجر به بروز اختلال در مهارت‌ کاربردشناختی می‌شود، اما آسیب به نیمکره راست با شدت اختلالات بیشتری همراه است. در رابطه با جایگاه آسیب مغزی و بروز اختلالات کاربرد شناختی نتایج پژوهش این نظر را تأیید می‌نماید که آسیب به لوب گیجگاهی و نواحی پیرامون آن در نیمکره راست مغز می‌تواند منجر به بروز اختلال در مهارت‌ کاربردشناختی شود. نتایج کلی پژوهش حاکی از نقش مهم نیمکره راست در پردازش مهارت کاربردشناختی نسبت به دیگر نواحی مغزی در نیمکره چپ است.

This study investigates pragmatic communication abilities in Persian-speaking adults with acquired brain injury (ABI), focusing on patients with right-hemisphere damage (RHD) and left-hemisphere damage (LHD). Pragmatic competence, crucial for interpreting metaphors and speech acts, depends on distributed neural networks, with increasing evidence highlighting the right hemisphere's critical role in processing non-literal meaning, inferencing, and social cues. Despite extensive research in non-Persian populations, evidence on pragmatic impairments in Persian speakers with focal brain injuries is limited. This study evaluates and compares pragmatic performance across RHD and LHD groups using the Montreal Protocol for the Evaluation of Communication (MEC). Results indicated that 60% of RHD patients exhibited pragmatic deficits compared to 30% of LHD patients, though group differences were not statistically significant (P > 0.05). Lesions in temporal lobe regions and adjacent areas were consistently associated with pragmatic impairments, suggesting a significant role for these regions in higher-level communication. These findings support a stronger role for the right hemisphere in non-literal meaning and contextual cue interpretation within the Persian language context.
